Enhance is funded by Health through Leeds Community Healthcare Trust, providing practical support to older and frail people, preventing readmission to hospital by offering person-centred, holistic support for people receiving community-based healthcare. We support individuals to attend Health HUBS, which are run every Thursday at Beeston Village Community Centre to empower them to improve their social connections, health, quality of life and wellbeing.
